İLKÖĞRETİM DÜZEYİNDEKİ SEREBRAL PALSİLİ ÇOCUKLARDA GÖVDE ETKİLENİM DÜZEYİNİN KATILIM, AKTİVİTE VE YAŞAM KALİTESİ ÜZERİNE ETKİSİNİN İNCELENMESİ

Abstract

ÖZET

Bu çalışmanın amacı iköğretim düzeyindeki SP’li çocuklarda gövde etkilenim düzeyinin aktivite, katılım ve yaşam kalitesi düzeyine olan etkisinin ve birbiri ile olan ilişkisinin incelenmesidir. Çalışmaya 7-12 yaş arası hemiparetik ya da diparetik spastik SP tanısı almış, Kaba Motor Fonksiyon Sınıflama Sistemi (GMFCS)’ne göre 1-2 ve 3 seviyesinde olan 16 hemiparetik SP’li (kız: 6, erkek:10; yaş ortalaması: 9,63±1,99), 16 diparetik SP’li (kız:10, erkek6; yaş ortalaması:9,38±1,74 ) olan toplamda 32 çocuk dahil edildi. Çocukların sosyodemografik verileri dosya bilgileri ve ebeveyneleri ile birlikte soru-cevap yöntemi ile belirlendi. Tüm çocuklar Gövde Etkilenim Ölçeği (GEÖ), Kaba Motor Fonksiyon Ölçütü (GMFM), Pediatrik Veri Toplama Aracı (PODCI)ve Çocuklar için Yaşam Kalitesi Envanteri- Serebral Palsi Modülü (PedsQL) ile değerlendirildi. Hemiparetik ve diparetik SP’li çocuklar arasında GEÖ toplam ve alt bölüm puanları, GMFM alt bölüm puanları, PODCI alt bölüm puanları ve PedsQL alt bölüm puanları arasında istatistiksel olarak anlamlı fark bulundu (p<0,05). Hemiparetik ve diparetik grupta GMFCS seviyesi ile GEÖ total puanları arasında negatif yönde orta derecede bir ilişki bulundu ( r: -,694, p:0,03; r: -,567, p:,0,02). Hemiparetik katılımcılarda GEÖ skorları ile GMFM-B alt bölüm skoru hariç diğer tüm bölüm skorlarında ilişki bulunurken, diparetik grupta tüm bölüm skorlarında ilişki tespit edildi. Hemiparetik ve diparetik grupta GEÖ skorları ile PODCI tüm alt skorları arasında ilişki bulunurken, hemiparetik grupta PedsQL- Konuşma ve İletişim alt bölüm skoru hariç tüm bölüm skorlarında ilişki bulunurken, diparetik grupta PedsQL tüm alt bölüm skorlarında ilişki tespit edildi. Çalışmamız sonucunda hemiparetik SP’li çocukların gövde kontrolünün diparetik SP’li çocuklara oranla daha iyi olduğu, GMFCS fonksiyonellik seviyesinin gövde kontrolünü önemli oranda etkilediği, gövde kontrolündeki yetersizliğin günlük yaşam aktivitelerindeki limitasyon, katılım kısıtlılıkları ve yaşam kalitesindeki azalma ile ilişkili olduğu, diparetik SP’li çocuklarda da üst ekstremite etkilenimi olduğu ve göz ardı edilmemesi gerektiği, fizyoterapistler tarafından Fizyoterapi ve Rehabilitasyon değerlendirme parametreleri ve tedavi sürecinde gövde kontrolünün geliştirilmesine yönelik egzersiz yöntemlerinin uygulanmasının, SP’li çocukların toplum içinde bağımsızlık düzeylerini artıracağı konusunda önemli rol oynayacağı düşünülmektedir.

ABSTRACT

The aim of this study is to examine the effect of trunk involvement level on activity, participation and quality of life in children with CP at the primary education level and their relationship with each other. In the study, 16 patients with hemiparetic CP (girls: 6, boys: 10; mean age: 7-12 years old) were diagnosed with hemiparetic or diparetic spastic CP and were at 1-2 and 3 levels according to Gross Motor Function Classification System (GMFCS). 9.63±1.99), 16 children with diparetic CP (female:10, boy6; mean age:9.38±1.74 years), a total of 32 children were included. The sociodemographic data of the children were determined by the question-answer method together with the file information and the parents. All children were evaluated with the Trunk Influence Scale (GES), Gross Motor Function Measure (GMFM), Pediatric Data Collection Tool (PODCI), and Children's Quality of Life Inventory-Cerebral Palsy Module (PedsQL). A statistically significant difference was found between the GES total and subdivision scores, GMFM subdivision scores, PODCI subdivision scores and PedsQL subdivision scores between children with hemiparetic and diparetic CP (p<0.05). In the hemiparetic and diparetic groups, a moderate negative correlation was found between GMFCS level and DES total scores (r: -.694, p:0.03; r: -.567, p:.0.02). In hemiparetic participants, there was a correlation between GES scores and all other department scores except for the GMFM-B subsection score, while a relationship was found in all department scores in the diparetic group. While there was a relationship between DES scores and all PODCI sub-scores in the hemiparetic and diparetic groups, there was a relationship in all sub-section scores except PedsQL- Speech and Communication sub-section score in the hemiparetic group, while a relationship was found in all PedsQL sub-section scores in the diparetic group. As a result of our study, trunk control of children with hemiparetic CP is better than children with diparetic CP, GMFCS functionality level affects trunk control significantly, lack of trunk control is associated with limitations in activities of daily living, limitations in participation and decrease in quality of life. It is thought that children with upper extremity are also affected and should not be ignored. It is thought that the application of Physiotherapy and Rehabilitation evaluation parameters and exercise methods to improve trunk control during the treatment process by physiotherapists will play an important role in increasing the level of independence of children with CP in the community.
