Reanimasyon Ünitesinde Yatan Hastaların Yakınlarının Gereksinimlerinin ve Psikolojik İyi Oluş Durumları ile İlişkisinin Değerlendirilmesi

Abstract

ÖZET Reanimasyon Ünitesinde Yatan Hastaların Yakınlarının Gereksinimlerinin ve Psikolojik İyi Oluş Durumları ile İlişkisinin Değerlendirilmesi Bu araştırma; reanimasyonda hastası olan hasta yakınlarının gereksinimlerinin ve psikolojik iyi olma durumları arasındaki ilişkinin değerlendirilmesi amacıyla kesitsel ve ilişki arayıcı olarak gerçekleştirildi. Ankara ili içinde bulunan bir üniversite hastanesinin reanimasyon ünitesinde tedavi gören hastaların yakınlarından 171 kişi araştırma kapsamına alındı. Araştırmada veriler Katılımcı Bilgi Formu, Yoğun Bakım Aile Gereksinim Envanteri ve Psikolojik İyi Olma Ölçekleri (PİOÖ) kullanılarak toplandı. Veri toplama araçları hasta yakınlarının hasta ziyaretlerini engellemeyecek şekilde 20 dakikalık süre zarfında yüz yüze görüşme yöntemiyle dolduruldu. Araştırmada elde edilen verilerin normal dağılım varsayımlarını sağlamaması sebebiyle karşılaştırmalı analizlerde Mann Whitney U testi, iki nicel değişken arasındaki ilişkiyi değerlendirmede Spearman korealasyon analizi kullanıldı. Katılımcıların yaş ortalaması 39,20±10,65 olup, %58,5’u kadındı. Gereksinim envanteri alt boyut puan ortalaması sırasıyla destek ve yakınlık 44,81±6,91, bilgi 41,75±4,38, güven 33,26±2,82, rahatlık 23,06±404’tür. PİOÖ toplam puan ortalaması 354,37±46,37 ve alt boyut puan ortalamaları diğerleriyle olumlu ilişkiler 63,04 10,89, bireysel gelişim 60,549,69, yaşam amaçları 59,638,10, çevresel hakimiyet 57,379,75, özerklik 57,118,75 ve öz kabul 56,709,08 olarak bulundu. Gereksinim envanteri destek ve yakınlık alt boyutu ile PİOÖ alt boyutları arasında anlamlı ilişki belirlenmedi. Gereksinim envanteri güven alt boyutu ile diğerleriyle olumlu ilişkiler (p=0,011) ve yaşam amaçları (p=0,009) alt boyutları arasında; Gereksinim envanteri bilgi alt boyutu ile özerklik (p=0,027), çevresel hakimiyet (p<0,001), bireysel gelişim (p<0,001) yaşam amaçları (p=0,005) alt boyutları ve PİOÖ toplam puan (p=0,002) arasında; Gereksinim envanteri rahatlık alt boyutu ile PİOÖ alt boyutlarından sadece diğerleriyle olumlu ilişkiler alt boyutu (p=0,048) arasında pozitif yönlü anlamlı ilişkiler saptandı. Sonuç olarak; psikolojik iyi oluşlar arttıkça gereksinimlerin ifade edilmesi ve açığa çıkma oranı da artmaktadır. Yoğun bakımda tedavi gören hastaların yakınlarının gereksinimleri düşünülerek bütüncül bakımın sağlanması önerilebilir. ABSTRACT Evaluatıon of the Association Between Needs of Relatives of Patıents in the Reanimation Unit and Their Psychological Well-Being This cross-sectional research was conducted to evaluate the relationship between the needs of relatives of patients in the reanimation unit and their psychological well-being. Overall, 171 relatives of patients who were treated in the reanimation unit of a university hospital in Ankara were included in the study. Data were collected using the participant information form, Intensive Care Family Needs Inventory, and Psychological Wellbeing (PWB) Scale. Data collection tools were filled by performing face-to-face interviews of 20-minute duration without interfering with relatives’ patient visitation. Because the data obtained in the research does not meet the normal distribution assumptions, the Mann-Whitney U test was employed for comparative analysis and Spearman’s correlation analysis was used to evaluate the relationship between two quantitative variables.The mean age of participants was 39.20 ± 10.65 years, and 58.5% of them were women. The average scores of the family needs inventory subscales were as follows: support and proximity 44.81 ± 6.91, information 41.75 ± 4.38, confidence 33.26 ± 2.82, and comfort 23.06 ± 404. The mean total score of the PWB scale was 354.37 ± 46.37, and the mean scores of subscales were as follows: positive relations with others 63.04 ± 10.89, individual progress 60.54 ± 9.69, life goals 59.63 ± 8.10, environmental dominance 57.37 ± 9.75, autonomy 57.11 ± 8.75, and self-acceptance 56.70 ± 9.08. No significant relationship was observed between the support and proximity subscales of the family needs inventory and the subscales of the PWB scale. Significant positive relationships were observed between the confidence subscale of the family needs inventory and the PWB subscales, namely positive relations with others (p = 0.011) and life goals (p = 0.009). In addition, significant correlation was noted between the information subscale of family needs inventory and the PWB subscales, namely autonomy (p = 0.027), environmental dominance (p < 0.001), individual progress (p < 0.001), life goals (p = 0.005), as wells as overall PWB scale score (p = 0.002). Moreover, a significant association was observed between the comfort subscale of the family needs inventory and positive relations with others (p = 0.048) subscale of the PWB. Consequently, an increase in psychological well-being results in an increased expression of requirements and positive relationships. Therefore, considering the needs of relatives of patients receiving intensive care treatment, the provision of holistic care can be recommended.
