HEMODİYALİZ HASTALARINDA ÖZ YETERLİLİK İLE ANKSİYETE VE DEPRESYON ARASINDAKİ İLİŞKİNİN İNCELENMESİ

Özet
Araştırma, hemodiyaliz hastalarında öz yeterlilik ile anksiyete ve depresyon arasındaki ilişkinin incelenmesi amacıyla tanımlayıcı-ilişkisel türde yapılmıştır. 04.07.2022-15.10.2022 tarihleri arasında Kastamonu Eğitim ve Araştırma Hastanesi Hemodiyaliz Birimi’nde tedavi alıp araştırmaya dahil edilme kriterlerine uyum gösteren 59 hasta ile gerçekleştirilmiştir. Veriler; Katılımcı Bilgi Formu, Genel Öz Yeterlilik Ölçeği (GÖYÖ), Beck Anksiyete Ölçeği (BAÖ) ve Beck Depresyon Ölçeği (BDÖ) ile toplanmıştır. Verilerin analizinde; tanımlayıcı istatistikler, Kolmogorov-Smirnov, Mann-Whitney U, Kruskal Wallis H, Ki-kare, Fisher Exact testleri, Pearson korelasyon analizi ve Cronbach alpha katsayısı kullanılmıştır. Katılımcıların GÖYÖ alt boyutları olan Çaba ve Direnç puan ortalaması 15,83±4,46, Yetenek ve Güven puan ortalaması 11,68±3,15, GÖYÖ toplam puanı ortalaması 27,51±7,27’dir. BAÖ alt boyutları olan Subjektif Kaygı puan ortalaması 5,61±5,27, Somatik Kaygı puan ortalaması 3,68±3,49, BAÖ toplam puanı ortalaması 9,29±7,79’dur. BDÖ toplam puanı 13,24±6,48 bulunmuştur. Çalışmaya katılan hastaların orta düzeyde öz yeterliliğe, hafif düzeyde ise anksiyete ve depresyona sahip oldukları tespit edilmiştir. GÖYÖ ile BAÖ ve BDÖ arasında negatif yönde orta düzey anlamlı bir ilişki bulunmuştur. BDÖ ve BAÖ arasında ise pozitif yönde orta düzey anlamlı bir ilişki saptanmıştır. Bu araştırma sonuçları doğrultusunda öz yeterlilik ile anksiyete ve depresyonun arasında ilişki olduğu, hemodiyaliz hastalarında öz yeterlilik düzeyi arttıkça depresyon ve anksiyete düzeylerinin azaldığı söylenebilir. Bu sebeple Konsültasyon Liyezon Psikiyatri Hemşiresi (KLPH) tarafından hastaların psikososyal açıdan düzenli olarak değerlendirilip hastalara ve hemodiyaliz hemşirelerine psikoeğitim ve danışmanlık verilmesi önerilebilir.
The research was conducted in a descriptive-relational type in order to examine the relationship between self-efficacy and anxiety and depression in hemodialysis patients. It was carried out with 59 patients who received treatment in Kastamonu Training and Research Hospital Hemodialysis Unit between 04.07.2022 and 15.10.2022 and complied with the inclusion criteria of the study. Data were collected with The Participant Information Form, the General Self-Efficacy Scale (GSES), the Beck Anxiety Inventory (BAI), and the Beck Depression Inventory (BDI). In the analysis of data; descriptive statistics, Kolmogorov-Smirnov, Mann-Whitney U, Kruskal Wallis H, Chi-square, Fisher Exact tests, Pearson correlation analysis and Cronbach alpha coefficient were used. The participants' GSES sub-dimensions, Effort and Resistance, mean score was 15,83±4,46, Talent and Confidence mean score was 11,68±3,15, GSES total score was 27,51±7,27. The mean score of Subjective Anxiety, which was the sub-dimensions of the BAI, was 5,61±5,27, the average of the Somatic Anxiety score was 3,68±3,49, and the total score of the BAI was 9,29±7,79. The BDI total score was 13.24±6.48. It was determined that the patients participating in the study had moderate self-efficacy and mild anxiety and depression. A moderately significant negative correlation was found between GSES and BAI and BDI. A moderately significant positive correlation was found between BDI and BAI. In line with the results of this study, it can be said that there is a relationship between self-efficacy and anxiety and depression, and as the self-efficacy level increases in hemodialysis patients, the depression and anxiety levels decrease. For this reason, it can be recommended that Consultation Liasion Psychiatric Nurse (CLPN) evaluate the patients regularly from a psychosocial point of view and provide psychoeducation and counseling to the patients and hemodialysis nurses."
