Öğe CAN PLACENTAL HISTOPATHOLOGICAL LESIONS BE A GUIDE TO MATERNAL AND NEONATAL OUTCOMES IN PATIENTS WITH PREECLAMPSIA?(Istanbul University Press, 2022) Atigan, A.; Kiliç, D.; Güler, T.; Karakaya, Y.A.Objective: The aim of this study was to investigate the predictive role of massive perivillous fibrinoid deposition (MPFD), syncytial knots, and accompanying histopathological features of placentas of preeclampsia (PE) on maternal and neonatal outcomes. Matherials and Methods: A retrospective clinicopathological study was conducted in a tertiary unit. In the study, 51 pregnant women admitted with PE and 55 normotensive healthy pregnant women matched for age and gestational age were compared. Information regarding clinical characteristics, neonatal findings, and placental properties such as syncytial knots, vascular structure density, placental area, volume, and weight) was retrieved. Results: Massive perivillous fibrinoid deposition, syncytial knots and decreased vessels in terminal villi were significantly frequent in the PE group compared to the controls. However, these histopathological findings were not associated with clinical and neonatal outcomes. Conclusions: Syncytial knot and perivillous fibrin deposition are significant microscopic findings of preeclampsia. However, the presence and amount of fibrin deposition were not correlated with perinatal outcome. © 2022 Istanbul Tip Fakultesi Dergisi.All rights reserved.Öğe The effect of vaginal douching on Pap smear(Pamukkale University, 2022) Gök, B.C.; Gök, S.; Sezer, S.D.; Atigan, A.Purpose: Vaginal douching damages vaginal flora and ecosystem. The disruption of the equilibrium of vaginal flora composition leads to many outcomes having a negative effect on women and newborn health. Materials and methods: 105 patients who applied vaginal douche were included in this study. All women taken of Pap smear test and the patients were informed in writing about the reasons why they must leave their vaginal douching habits, and a second Pap smear appointment day not earlier than 30 days were given. After taken of patients first Pap smears, none of them took an infection treatment. Pap smear results taken before and after vaginal douche have been compared. Results: It was seen in the detailed comparison of Pap smear outcomes that the rate of patients with violent inflammation decreased from 11.4% to 1.9%. The decrease in violent inflammation was found statistically significant (p=0.006). When the patients with infection were compared regardless of any discrimination of slightly or violent; the rate of the cases with typical infection findings was seen to decrease from 48.5% to 31.4% and found statistically significant (p=0.01). Conclusions: Vaginal flora is a very complex and dynamic component and is under the influence of many factors. Vaginal douching damages vaginal flora structure and balance, and adversely affects women's health in a wide range from a simple vaginal infection to cervical neoplasia. © 2022, Pamukkale University. All rights reserved.Öğe Microarray expression results of VEGF, YAP1 and PTEN immunostains in preeclampsia cases(Taylor and Francis Ltd., 2023) Atigan, A.; Karakaya, Y.A.; Kiliç, D.; Guler, O.T.We aimed to evaluate the expression of YAP1, PTEN, VEGF in the placentas of patients with preeclampsia and placentas of healthy pregnant women for trophoblast invasion, which is similar to cancer etiopathogenesis. The placentas of 70 women who gave birth, including 30 preeclampsia and 40 healthy controls, were evaluated. YAP1, PTEN and VEGF immunohistochemical staining were performed using the microarray method on placental tissue. The mean ± standard deviation for YAP1, PTEN and VEGF intensity were; 1.57 ± 0.71,2.59 ± 0.80, 1.61 ± 0.59, respectively. PTEN intensity was statistically significantly lower in the preeclampsia group than in the control group (2.37 ± 0.99 vs 2.75 ± 0.58, p = .049). There was no difference between the groups in terms of YAP1 and VEGF staining (p > .05). The etiopathogenesis of preeclampsia is still unclear. However, since trophoblast invasion and endothelial repair have similar aspects with cancer mechanisms, both preeclampsia and cancer studies are progressing by supporting each other. Our study is a prototype study showing that large-participation studies can be carried out easily by using the microarray method as an economic model. © 2023 Taylor & Francis.
