Öğe Onychomycosis and erectile dysfunction. Does common etiology cause togetherness?(2021) Baran, Özer; Aykaç, Aykut; Kaya, Coskun; Aktaş, HabibullahAim: We tried to determine the frequency of co-occurrence of erectile dysfunction (ED) and Onychomycosis (OM). Material and Methods: The sociodemographic form and IIEF-5 form were filled by the male patients who were admitted to the Urology Polyclinic of Karabük University Medical Faculty with the complaints of ED between April 2019 - April 2020. The patients with ED were included in the study. All the male patients were divided into three groups as mild, moderate and severe according to ED severity. The dermatologist (H.A) examined the presence of OM by taking samples from the toenails of the male patients. Results: Looking at the IIEF-5 scores of 95 patients accepted to the study, it was found that 22 (23.1%) had mild, 20 (21%) moderate and 53 (55.9%) severe ED. Smoking history was higher in the severe ED group (p <0.001). It was found that a total of 32 (33.7%) patients had OM. However, there was no relationship between the severity of ED and the presence of OM (p> 0.114). Conclusion: Erectile dysfunction and OM disease can often be seen together. However, no relationship was found with the severity of ED.Öğe Penile degloving and dorsal dartos flap rotation approach for the management of isolated penile torsion(2016) Aykaç, Aykut; Baran, Özer; Yapıcı, Onur; Aygün, Bülent Alper; Aydın, Cemil; Çakan, MuratAmaç: Penil torsiyon genellikle saat yönünün tersine doğru görülen penis gövdesi veya glansın rotasyonel anomalisidir. İzole penil torsiyon onarımında penil degloving ve dorsal dartos fleb rotasyonu sonuçlarını değerlendirmek.Gereç ve yöntemler: Ocak 2013-Aralık 2014 tarihleri arasında sünnet amaçlı polikliniğimize başvuran hastaların yapılan işlem sırasındaki değerlendirmelerinde 5 olguda izole penil torsiyon izlendi. Bipolar koter eşliğinde yapılan sünnet işlemini takiben, penil degloving yapılan hastalarda penil torsiyonda düzelme olup olmadığı kontrol edildi. Yeterli düzelme gözlenmeyen olgularda hazırlanan dorsal dartos fleb ile torsiyon düzeltildi. 2 olguda dartos flebi, 3 olguda ise penil degloving ile penil torsiyonda düzelme sağlandı. Bulgular: Hastaların ortalama yaşı 5,6 yıl , ortalama operasyon süresi 12 dk idi. Dorsal dartos fleb ve penil degloving yapılan gruplarda ortalama operasyon süresi sırasıyla 17,5 dk ve 8,3 dk idi. Hastaların ameliyat öncesi ortalama 50° (30°-70°) penil torsiyonu mevcut idi. Dorsal dartos flebi kullanılanlarda ortalama torsiyon derecesi 65° iken sadece degloving yapılanlarda ise 40° idi. Ameliyat sonrası takiplerde dorsal dartos flebi kullanılan 1 olguda 10 derecenin altında torsiyon gözlendi. Diğer hastalarda rezidüel torsiyon izlenmedi. Sonuç: Sünnet işlemi sırasında izole penil torsiyon olgularının gözden kaçırılmaması gerekmektedir. Düşük ve orta dereceli torsiyonlarda penil degloving ve dartos fleb rotasyonu ile başarılı sonuçlar alınması mümkün gözükmektedirÖğe What is the ideal age of circumcision for wound healing time?(2016) Aykaç, Aykut; Yapıcı, Onur; Baran, Özer; Oğuz, Ural; Çakan, MuratObjective. Circumcision is practiced worldwide. Currently, the effect of age at the time of circumcision on wound healing is not fully known. This study aimed to determine the effect of age at the time of circumcision on wound healing. Methods. The study included 382 male patients aged ?16 years that were circumcised between January 2014 and December 2014. Data for 345 patients that were followed-up regularly were evaluated retrospectively. Circumcision was performed using a bipolar diathermic knife. Circumcision wounds were considered healed when the 2 suture lines were observed to be completely apposed. Wound healing time was analyzed according to age at the time of circumcision. Results. Mean age of the patients was 7.2 years (range; 6 days-16 years). Mean duration of surgery was 327.5 sec and mean healing time was 4.3 days. The patients were classified according to age as group 1 (0-2 years; n=114; 32.9%), group 2 (3-6 years; n=60; 17.3%), and group 3 (7-16 years; n=171; 49.8%). Mean duration of surgery in groups 1-3 were 280.9 sec, 320.2 sec and 356.5 sec; respectively, and healing times were 4.1, 4.2 and 4.5 days; respectively. Healing time was significantly longer in group 3 than in groups 1 and 2 (p<0.05). There was no significant difference in healing time between groups 1 and 2 (p>0.05). Conclusion. Wound healing time was shorter in the patients aged 0-6 years than in those aged 7-16 years.
