    Acupuncture Effects on Blood Parameters in Patients with Fibromyalgia

    Objectives: Acupuncture reduces pain and symptoms significantly in fibromyalgia (FM). Currently, there are no laboratory markers to monitor treatment effectiveness. This study measured differences in biomarkers and test scores of patients with FM before and after acupuncture. Materials and Methods: This retrospective study involved 102 females previously diagnosed with FM at a rheumatology clinic, who had received various medical treatments. Patients who did not receive treatment for symptom management of FM for the last 3 months were included. C-reactive protein (CRP) and complete blood count (CBC), and Fibromyalgia Impact Questionnaire (FIQ) visual analogue scale (VAS) scores before and after the acupuncture sessions were obtained from the patients' files. Neutrophil/lymphocyte ratio (NLR), platelet/lymphocyte ratio (PLR), mean platelet volume/lymphocyte ratio (MPVLR), monocyte/lymphocyte ratio (MLR) were calculated. It was determined if there were statistically differences in these parameters. A correlation analysis was also performed. Results: There were statistically significant decreases in CRP, NLR, PLR, and MPVLR values as well as VAS and FIQ scores in the patient post acupuncture. NLR was positively correlated with disease impact and CRP after completion of acupuncture. Conclusions: The decreasing tendency of the markers from the CBC after acupuncture were correlated with FM impact. These results show effects of acupuncture on biomarkers. Prospective randomized controlled trials on how acupuncture affects blood parameters in patients with FM are needed.

    Comparison of stress coping attitudes in caregivers for cancer and non-cancer patients on palliative care

    Introduction: Changes in the social and emotional state of patient caregivers are associated with the individual care burden of the patient rather than the diagnosis of the patient. Our aim was to compare the stress levels and coping attitudes of caregivers who give palliative care to patients with cancer and non-cancer. Material and methods: This non-randomised, controlled study included caregivers providing palliative care to patients diagnosed with cancer and non-cancer in the palliative care unit. The participants were evaluated in respect of age, gender, duration of caregiving, relationship to the patient, marital status, level of education, occupation, psychiatric history, chronic disease history, and smoking status using a data collection form, and the means of coping with stress with the Coping Orientation to Problems Experienced (COPE) scale. Results: Evaluation was made of a total of 80 participants comprising 40 (50%) caregivers of patients diagnosed with cancer and 40 (50%) caregivers of patients not diagnosed with cancer. Female gender, smoking, and caregiving duration were significantly higher (p = 0.009, p = 0.001, p = 0.001, respectively) and educational level was lower (p = 0.022) in the caregivers of non-cancer patients. In the subscales of COPE, the scores of suppressing other activities (p < 0.001), mental disengagement (p = 0.001), and emotion-focussed coping (p = 0.007) were higher in the group of caregivers of non-cancer patients. Conclusions: Because the duration of the cancer patient's need for care is shorter than that of non-cancer patients, this may cause the caregivers of cancer patients to be less able to adapt to the stressful process they experience than the caregivers of non-cancer patients.

    The effect of blood urea nitrogen/creatinine and blood urea nitrogen/albumin ratios on the prognosis and mortality of patients in the palliative care service

    Introduction: In this study we aimed to evaluate the effects of the blood urea nitrogen/creatinine ratio (BUN/BCR) and the BUN/albumin ratio (BAR) on the prognosis and mortality in patients in the palliative care service.Material and methods: Patients in the palliative service were included in the study. Sociodemo-graphic data of the patients such as age, gender, chronic diseases, discharge from the service, length of stay in the service, BUN, creatinine, albumin, C-reactive protein (CRP), and haemogram parameters were recorded retrospectively from patient files and the hospital automation system. BCR and BAR values were calculated.Results: A total of 209 patients were included in the study. 57.4% of them were male, and 44.9% were >= 81 years of age. While 78.0% of the patients were discharged alive, 22.0% of them died. When the blood test results of the patients who were discharged from the palliative service alive and those who died were compared, it was seen that the CRP, BUN, BCR, and BAR levels were high, and the albumin levels were low in the patients who died. A positive correlation was ob-served between BCR and BAR levels in patients in the palliative service. It was observed that as BAR and BCR levels increased, the length of stay in the service, BUN, creatinine, and CRP levels increased, while albumin levels decreased.Conclusions: In our study, a positive correlation was found between BCR and BAR parameters, hospitalization time, and mortality status of patients in the palliative service with various systemic diseases. In this case, it can be said that the BCR and BAR ratios have value in predicting the out-comes and mortality in patients.

    Effect of ozone therapy on neutrophil/lymphocyte, platelet/lymphocyte ratios, and disease activity in ankylosing spondylitis: a self-controlled randomized study

    This retrospective self-controlled randomized study was carried out with the participation of 53 patients diagnosed with ankylosing spondylitis according to the modified New York criteria. The patients who did not receive medical treatment or did not change their medical treatment within the last 6 months were included in the study. There was a statistically significant decrease in the patients' neutrophil/lymphocyte ratio, platelet/lymphocyte ratio, monocyte/lymphocyte ratio, C-reactive protein, Visual Analog Scale, Bath Ankylosing Spondylitis Functional Index, and Bath Ankylosing Spondylitis Disease Activity Index scores measured after ozone therapy. There was a positive correlation between neutrophil/lymphocyte ratio, platelet/lymphocyte ratio, mean platelet volume/lymphocyte ratio, monocyte/lymphocyte ratio and C-reactive protein, Visual Analog Scale, Bath Ankylosing Spondylitis Functional Index, Bath Ankylosing Spondylitis Disease Activity Index before and after ozone therapy. Our study revealed that the changes in the decreasing tendency of the markers measured in complete blood count after ozone therapy were correlated with the disease activity, which can contribute to understand the effect of ozone therapy on biomarkers.

    Evaluation of Causes Application on Medical Board

    Aim: Disabled patients are admitted to the medical board to determine the disability rate in order to use the occupational, health and social rights in Turkey. According to the law, over 40% of individuals with disabilities, based on the severity of their disability and illness rate, have different rights. We aimed to contribute to epidemiological data on people with disabilities in our city. Methods: Between January 2014 and December 2014 the records of patients were admitted to the medical board of Karabuk University Training and Research Hospital, and they were analyzed retrospectively. Results: In the medical board in January 2014-December 2014, 2637 cases were evaluated. 1554 of these patients were male (58.9%), 1083 women (41%). With the increase of age, the frequency of application increased, too. Maximum application was in the group who are above 65 years of age. The numbers of cases by over 40% points are 1834 people (69.5%). 1055 of them were men (40%), 779 were women (29.5%). When the cases were evaluated according to the distribution of branches, 976 cases (37%) in musculoskeletal system were identified. The least disability was in Obstetrics and Gynecology with 8 cases (0.3%). Conclusion: Quality of life of the disabled population and the quality of services which are offered to them are closely related to the level of development of the society. In order to improve the life quality of disabled people, regional and country wide data of these people are needed.

    Evaluation of Knowledge and Attitudes of Cancer Patients on Vitamin and Mineral Supplements

    Background: The use of vitamin and mineral supplements (VMS) in different patient populations, especially in cancer patients, has increased in recent years. In this study, we aimed to compare the knowledge and attitudes of cancer patients and healthy population about VMS use. Materials and Methods: A questionnaire was administered to cancer patients and healthy control group to get insights into their sociodemographic characteristics and knowledge and attitudes about VMS use. Results: Of the 778 participants, 217 (27.8%) were cancer patients. The use of VMS was higher in the cancer patients (60.4%) than in the control group (32.8%). The rates of VMS use were higher in the patients with cancer at an advanced stage. The rate of using Vitamin B12, Vitamin D, folic acid, iron, and calcium was higher in the cancer patients compared to the healthy population. The use of Vitamin B complex and folic acid was higher in the patients with gastric cancer, and the use of Vitamin D and calcium was higher in the patients with breast cancer compared to other cancer types. Conclusion: The use of VMS was found to be higher in the cancer patients than in the control group. Living in a city center and having an advanced-stage cancer were seen as the predictive factors for the use of VMS in cancer patients. It was observed that the cancer patients used VMS frequently because the physician in charge recommended and they saw a positive effect after using VMS. The control group obtained the information about VMS through the media. It is possible to have more positive results if VMS is used under a physician's control and indication.

    Evaluation of multiple drug use in patients with type 2 diabetes mellitus

    Objective Multiple drug use (Polypharmacy) is common in Diabetes Mellitus (DM) patients. The purpose of this study was to evaluate the presence of polypharmacy and comorbid conditions in patients with DM. Method The sociodemographic data, comorbidity diseases, and prescription records of 607 patients diagnosed with type 2 DM were retrospectively analyzed. Polypharmacy was defined as the use of five or more different drugs. Results The mean number of drugs used by the DM patients was 6.7 +/- 2.5. It was observed that 77.9% of the DM patients had polypharmacy. The mean number of drugs used by the patients in the polypharmacy group was 7.7 +/- 1.7. The most common comorbidities in DM patients were diseases of the musculoskeletal system. The use of drugs for musculoskeletal diseases and the number of drugs were statistically higher in female patients than in male patients. In the DM patients, polypharmacy was higher in the females, those older age, those having a longer history of DM disease, and those having a comorbid disease. Conclusion The total number of drugs used by the DM patients showed the presence of polypharmacy. Advanced age, long disease duration, female gender, and presence of comorbidities were predictive factors for polypharmacy in diabetic patients. Before starting additional medication for DM patients, it is necessary to pay attention to the interaction of the drugs to be used and to plan prescriptions considering the medications used by the patient continuously.

    Evaluation of the Effect of Comorbid Bronchiectasis on Quality of Life in Patients with Chronic Obstructive Pulmonary Disease

    Objective: The objective of this study was to evaluate the effect of comorbid bronchiectasis on quality-of-life in patients with Chronic Obstructive Pulmonary Disease (COPD). Method: 103 patients were diagnosed with COPD were included in our study. Spirometric measurements were made. The following measurement tools were used to collect data: 6-Minute Walk Test (6MWT), Modified Medical Research Council (mMRC) dyspnea scale, COPD Assessment Test (CAT), St. George Respiratory Questionnaire (SGRQ), and Short Form 36 (SF-36) quality-of-life questionnaire. Furthermore, all the patients' high Resolution Computed Tomography (HRCT) images for the last three years were included in the study. Result: 93.2% of the patients were male, with a mean age of 64.79 +/- 9.35 years. It was found that SGRQ and SF-36 quality-of-life scores decreased by half in all the patients compared to normal ones. The volume/forced vital capacity (FEV1/FVC) values in the first second of forced expiration were significantly lower in the group with bronchiectasis. A significant correlation was found to exist between the patients' mMRC dyspnea scale and CAT scores, 6MWT distances, and all subscales of SGRQ and SF-36. In addition, a significant correlation was also found to exist between FEV1 values and all subscales of SGRQ, and between subscales of SF-36. Conclusion: In our study, when we compared the quality-of-life scores of the patients with COPD and bronchiectasis with those with COPD alone, we found that the quality-of-life of both groups was impaired, but there was no significant difference between them.

    The relationship between depression, anxiety, and pain catastrophising in cancer patients

    Introduction: Catastrophising is a person's view of an unreasonable belief and a worse situation than exaggerating its consequences. It is defined as individuals believing that their current condition and physical discomfort will worsen each time or that something will be worse than it actually is. The aim of this study was to evaluate the relationship between depression and anxiety and pain catastrophising in cancer patients. Material and methods: Fifty-five cancer patients who were followed and treated in the Oncology Outpatient Clinic were included in the study. The age, gender, marital status, occupation, and psychiatric history of the participants were recorded. A socio-demographic data form, Beck Depression Inventory (BDI), Beck Anxiety Inventory (BAI), and the Pain Catastrophising Scale (PCS) were applied to the participants. Results: The mean age of the participants was 59.2 +/- 12.7 years, and 40% were female. When the total BDI and BAI scale scores of cancer patients were evaluated, it was seen that they were not depressed in terms of mean value (p = 0.112), but they were in the anxiety scale (p < 0.05). There was a positive correlation between depression and anxiety, as well as depression and anxiety and pain catastrophising (p < 0.001, r = 0.782). While the PCS subscales helplessness and rumination scores were significantly higher in cancer patients with depression and anxiety (p < 0.001), the magnification score was significantly higher in cancer patients with anxiety (p < 0.001). The Pain Catastrophising Scale total score increased with increasing BDI and BAI severity (p < 0.001), while the BDI and BAI scores were found to be significantly higher in females than males (p < 0.001). Conclusions: It was found that cancer patients were generally not depressed but were anxious, and the catastrophising of pain increased with increasing depression and anxiety severity. Health professionals giving care to cancer patients need to be alert to signs of psychological distress in patients experiencing pain.

    Self-esteem, metacognition, and coping strategies in cancer patients: A case-control study

    Background: Self-esteem refers to a person's positive and negative attitudes towards the self, and metacognition is an upper system providing awareness and direction of events and mental functions. Coping refers to the specific and psychological efforts used to deal with stressful events or the negative effects of the agents of these. The aim of this study was to evaluate self-esteem, metacognition status and coping attitudes in patients with cancer, which is known to have severely destructive psychological effects. Materials and Methods: Fifty adult cancer patients who were followed up in the medical oncology clinic between July 2018 and June 2019 and 50 age- and gender-matched healthy controls as control group were included in this study. All the participants were applied with a sociodemographic data form, the Rosenberg self-esteem scale, the Metacognition Assessment Scale, and the Copying Orientation to Problems Experienced (COPE) inventory, and their results were compared between the groups. Results: The groups comprised 50% females with a median age of 58 (33-82) years. The values related to the degree of participation in discussions, problem-focused coping, active coping, planning, and state of emotional vulnerability were low in the cancer patient group compared to the control group (P < 0.005 for all). The sustaining of their self-image, feeling threatened in interpersonal relationships, and degree of daydreaming were higher, and in the metacognition tests, the positive beliefs related to anxiety, uncontrolled or dangerous negative thoughts, nonfunctional coping, religious coping, joking, reckless behavior, substance use, denial, and mental disengagement scores were higher (P < 0.05 for all). Conclusion: Self-esteem was lower in cancer patients and upper level cognitive functions and problem-focused coping were determined to be worse compared to healthy controls. In the light of these results, psychosocial support given to cancer patients in this respect could contribute to quality of life and social conformity.

    Seroprevalence of Viral Hepatitis According to Age Groups in Individuals Applying to a University Hospital Family Medicine Polyclinic

    Objective: As in all over the world, viral hepatitis is an important public health problem in our country. The aim of this study was to investigate the seroprevalence of hepatitis A, hepatitis B and hepatitis C in patients who applied to the Family Medicine out patient clinic at a university hospital. Methods: The data of the patients who were admitted to Karabuk University Education and Research Hospital Family Medicine out patient clinic for any reason between January 2016-December 2018 and who were asked for hepatitis markers were included in the study. Age, gender, demographic data, HBsAg, AntiHBs, AntiHCV, AntiHAVIgM and AntiHAVIgG ELISA test results were evaluated retrospectively. Results: Among 4297 people included in the study, 48.4% were female and 51.6% were male. The mean age was 36.9 +/- 14.7 (2-91) years. In our study, seropositivity was found in 1%, 45%, 0.8%, 0.1% and 57% of HBsAg, AntiHBs, AntiHCV, AntiHAVIgM and AntiHAVIgG tests, respectively. AntiHBs seropositivity rate was highest in the 0-29 age group. Conclusions: The rate of HBV, HCV and HAV infection was found to be low in the applicants of family medicine. With the inclusion of the HBV vaccine in the national vaccination program, AntiHbs positivity was significantly higher in the younger group.

    Türkiye’de tıp fakülteleri temel tıp bilimlerinin ders saatleri ve akademisyen sayılarının karşılaştırılması

    Giriş: Tıp eğitimi uzun yıllar tartışılmış, çeşitli standardizasyon tanımlamaları yapılmış ve teknolojideki gelişmelere paralel olarak yeni eğitim metotları gündeme gelmiştir. Başarılı bir hekimolabilmek klinik deneyimin yanı sıra çok iyi bir teorik alt yapı gerektirmektedir.Amaç: Türkiye’de eğitim veren tıp fakültelerinde temel tıp bilimi eğitiminde ders saatlerinin veakademisyen sayılarının üniversitelere göre değişiminin incelenmesi amaçlanmıştır.Gereç ve Yöntem: Türkiye’de eğitim veren 82 tıp fakültesinden eğitim öğretim programlarına internet üzerinden ulaşılabilen 41 fakülteninders saatleri kaydedildi. Temel tıp bölümlerindeki öğretim üyelerininsayıları ve temel tıp bilimleri ders saatleri belirlendi. Analizler tanımlayıcı istatistikler kullanılarak gerçekleştirildi.Bulgular: Eğitim saati en fazla olan dersler Biyokimya (149 saat) ve Patoloji (127 saat) ve Mikrobiyoloji (124 saat) olurken tıp tarihi ve deontoloji dersi ile davranış bilimleri dersi en az eğitim saati olan derslerdi.En fazla temel tıp bilimleri öğretim elemanı Ankara Üniversitesi’nde(108 kişi) yer alırken Amasya Üniversitesi’nde hiç öğretim elemanı olmadığı belirlendi.
