COVID-19 DÖNEMİNDE DOĞUM YAPAN KADINLARIN POSTPARTUM DEPRESYON VE ANNE BEBEK BAĞLANMA DURUMLARININ İNCELENMESİ: KARABÜK İLİNDE HASTANE TABANLI BİR ARAŞTIRMA

Özet
Çalışma COVID-19 döneminde doğum yapan kadınların postpartum depresyon ve anne bebek bağlanma durumlarının incelenmesi amacıyla planlanmıştır. Kesitsel tipteki çalışmada, veriler 20 Mart 2022- 20 Temmuz 2022 tarihleri arasında Karabük Eğitim ve Araştırıma Hastanesi’nde (KEAH) toplanmıştır. Araştırmanın evrenini, KEAH’nde 2021 yılında doğum yapan 1291 anne, örneklemini ise prevalans %25 (Türkiye’de postpartum depresyon görülme sıklığı), hata payı %5 ve %95 güven aralığı alınarak yapılan hesaplamada doğum sonu dönemde olan 236 anne oluşturmuştur. Araştırmanın bağımlı değişkenleri, COVID-19 korkusu, postpartum depresyon ve anne bebek bağlanması puanlarıdır. Araştırma için, Kovid-19 Korkusu Ölçeği, Edinburgh Postpartum Depresyon Ölçeği ve Maternal Bağlanma Ölçeği kullanılmıştır. Araştırmanın bağımsız değişkenleri ise anneye ait sosyo-demografik özellikler, obstetrik özellikler, bebek özellikleri ve COVID-19 parametreleridir. Araştırma verilerinin temel analizlerinde (korelasyon, frekans) bilgisayar destekli veri analiz programı kullanılmıştır. İkili değişkenler için independent sample T testi, üç ve üzeri değişkenler için Tek Yönlü Varyans Analizi (ANOVA/WELCH) kullanılmıştır. Çalışmada annelerin %50,8’inin COVID-19 geçirdiği ve %22’sinin gebelik döneminde geçirdiği görülmüştür. Annelerin %14,8’inin aileden herhangi birini COVID-19 nedeniyle kaybettiği görülmüştür. Annelerin Kovid Korkusu Ölçeği puanının 18,97±8,68; Edinburgh Postpartum Depresyon Ölçeği puan ortalamasının 4,02±5,16 ve Maternal Bağlanma Ölçeği ise 102,47±4,12 puan olduğu saptanmıştır. Gebelikte stres yaşamanın Maternal Bağlanma Ölçeği puanlarını 2,28 kat azalttığı ve Edinburgh Depresyon Ölçeği puan ortalamasında oluşan 1 birimlik artışın maternal bağlanmayı 0,19 kat azalttığı görülmüştür. Çalışma sonucunda COVID-19 korkusunun annelerin doğum sonrası depresyon durumunu etkilediği, annelerin maternal bağlanma sorunu yaşadığı ve anne bebek bağlanmasının etkilendiği görülmüştür.
The study was planned to examine the postpartum depression and mother-baby attachment status of women who gave birth during the COVID-19 period. In the cross-sectional study, data were collected at Karabuk Training and Research Hospital (KEAH) between 20 March 2022 and 20 July 2022. The population of the study consisted of 1291 mothers who gave birth in 2021 with CIAD, and the sample consisted of 236 mothers who were in the postpartum period in the calculation made by taking the prevalence of 25% (prevalence of postpartum depression in Turkey), the margin of error of 5%, and the 95% confidence interval. The dependent variables of the study are fear of COVID-19, postpartum depression and mother-infant attachment scores. For the research, Kovid-19 Fear Scale, Edinburgh Postpartum Depression Scale and Maternal Attachment Scale were used. The independent variables of the study are maternal socio-demographic characteristics, obstetric characteristics, infant characteristics and COVID-19 parameters. A computer aided data analysis program was used in the basic analysis of the research data (correlation, frequency). Independent sample T test was used for binary variables and One Way Analysis of Variance (ANOVA/WELCH) was used for three or more variables. In the study, it was seen that 50.8% of the mothers had COVID-19 and 22% of them had it during pregnancy. It was observed that 14.8% of the mothers lost any member of the family due to COVID-19. The mothers' Kovid Fear Scale score was 18.97±8.68; The Edinburgh Postpartum Depression Scale mean score was 4.02±5.16, and the Maternal Attachment Scale was 102.47±4.12 points. It was observed that experiencing stress during pregnancy decreased the Maternal Attachment Scale scores by 2.28 times, and a 1-unit increase in the Edinburgh Depression Scale score average reduced maternal attachment by 0.19 times. As a result of the study, it was seen that the fear of COVID-19 affected the postpartum depression of the mothers, that the mothers had maternal attachment problems and that mother-infant attachment was affected."
