TOTAL DİZ ARTROPLASTİSİ AMELİYATI YAPILAN HASTALARIN CERRAHİ KORKU DÜZEYLERİ İLE AMELİYAT SONRASI ANKSİYETE VE MOBİLİZASYON ARASINDAKİ İLİŞKİNİN İNCELENMESİ

Özet
Total diz artroplastisi ameliyatı yapılan hastaların cerrahi korku düzeyleri ile ameliyat sonrası anksiyete ve mobilizasyon arasındaki ilişkinin incelendiği bu çalışma tanımlayıcı olarak yapıldı. Karabük ilinde yer alan bir eğitim ve araştırma hastanesinin Ortopedi ve Travmatoloji Servisinde yatan ve total diz artroplastisi ameliyatı olan 93 hasta çalışma kapsamına alındı. Çalışmanın verileri hastaların sosyodemografik özelliklerini içeren kişisel bilgi formu, “Cerrahi Korku Ölçeği”,“Durumluk Sürekli Kaygı Ölçeği” ve mobilizasyon soru formu ile toplandı. Veriler, tanımlayıcı istatistikler, Shapiro-Wilk, Kolmogorov-Smirnov ve Anderson-Darling testleri, Independent Samples T-Test, Mann Whitney U testi, Pearson ve Spearman’s Rho korelasyon katsayısı kullanılarak analiz edildi. Araştırmaya katılan hastaların yaş ortalaması 66,8 ± 7,8 yıl olup %82,8’i kadındı. Hastaların Cerrahi Korku Ölçeği puan ortalaması 21,2 ± 23,3; Durumluk Kaygı Ölçeği puan ortalaması 38,0±9; Sürekli Kaygı Ölçeği puan ortalamasının 44,2 ± 4,3 idi. Hastaların ameliyattan sonra ayağa kalkma isteği 6,6 ± 2,4 olarak belirlendi. Hastaların gün içerisindeki mobilizasyon sayısının 5,2 ± 2,2 olduğu, gün içerisinde mobilizasyon sırasında ağrı düzeyinin 6,1 ± 1,6 olduğu saptandı. Hastaların Cerrahi Korku Ölçeği ve alt boyut puanları ile Durumluk Kaygı Ölçeği puanları arasında istatistiksel olarak anlamlı bir ilişki olduğu tespit edildi (p<0,001). Hastaların Durumluk Kaygı Ölçeği puanı ile ameliyattan sonra ayağa kalkma isteği puanı arasında istatistiksel olarak anlamlı bir ilişki olduğu belirlendi (p=0,039). Araştırma sonuçları doğrultusunda; hastaların anksiyete düzeyini azaltmak ve mobilizasyon sayılarını arttırmak için protokollerin geliştirilmesi, Ortopedi ve Travmatoloji Servisinde çalışan hemşirelerin konu ile ilgili hastalara eğitim ve danışmanlık yapması önerilmektedir.
This descriptive study was conducted to examine the relationship between surgical fear levels and postoperative anxiety and mobilization in patients who underwent total knee arthroplasty surgery.93 patients who were hospitalized in the Orthopedics and Traumatology Service of a training and research hospital in Karabuk and underwent total knee arthroplasty were included in the study.The data of the study were collected with the personal information form containing the sociodemographic characteristics of the patients, the ""Surgical Fear Scale"", the ""State-Trait Anxiety Inventory "" and the mobilization questionnaire.Data were analyzed using descriptive statistics, Shapiro-Wilk, Kolmogorov-Smirnov and Anderson-Darling tests, Independent Samples T-Test, Mann Whitney U test, Pearson and Spearman's Rho correlation coefficient.The mean age of the patients participating in the study was 66.8 ± 7.8 years, and 82.8% of the patients were women.The mean surgical fear scale score of the patients was 21.2 ± 23.3; The state scale mean score was 38.0 ± 9; trait anxiety scale mean score was 44.2 ± 4.3.The patients' willingness to stand up after the operation was determined as 6.6 ± 2.4. It was determined that the number of mobilizations of the patient’s during the day was 5.2 ± 2.2, and the level of pain during mobilization during the day was 6.1 ± 1.6.It was determined that there was a statistically significant difference between the patients' Surgical Fear Scale and sub-dimension scores and State Anxiety Scale scores (p<0.001).It was determined that there was a statistically significant difference between the patients' state anxiety scale score and their willingness to stand up after surgery(p=0.039).In line with the research results; It is recommended that protocols should be developed to reduce the anxiety level of patients and increase the number of mobilizations, and that nurses working in orthopedics and traumatology services should provide education and counseling to patients on the subject."
